Position Summary

The CORE Direct Support Professional I will assist in providing direct care and support to individuals with disabilities, helping them achieve their personal goals. The position will assist individuals in achieving their desired outcomes, maintain required documentation, and provide transportation to various community settings. This is an entry-level role focused on learning and applying key aspects of person-centered support.

Essential Job Duties

  • Assist individuals in achieving their individual goals through action plans and document progress.
  • Participate in the Person-Centered Support Planning process.
  • Provide transportation to individuals to and from community settings.
  • Assist individuals with personal care, including meals, hygiene, and health maintenance.
  • Help facilitate relationships and encourage participation in community activities.
  • Provide emergency/crisis intervention as needed for medical or behavioral situations.
  • Support individuals in a friendly, respectful, and caring manner.
  • Adhere to the agreed-upon work schedule, report for work punctually, and ensure reliable attendance.
  • Accurately record hours worked on the timesheet.
  • Engage individuals in activities that promote life skills and teamwork.
  • Other duties as assigned.

Qualifications

  • High school diploma or equivalent
  • 0 – 2 years of experience working with individuals with developmental disabilities preferred
  • Must have a valid driver’s license and driving record acceptable to Cottonwood’s insurance carrier

Knowledge, Skills, and Abilities

  • Ability to assist individuals with mobility and daily activities, including lifting and transferring
  • Basic computer skills, including data entry and email access
  • Effective communication skills to interact with individuals, co-workers, and supervisors
  • Ability to manage challenging behaviors and provide crisis intervention when necessary
  • Patience, compassion, and empathy when working with individuals with disabilities
  • Adaptability and problem-solving skills to address various situations
  • Ability to maintain a safe and healthy environment for individuals served
  • Ability to foster community engagement and promote independence through meaningful activities

Physical Demands

  • Frequently required to assist individuals with mobility and daily activities, including lifting and transferring individuals
  • Frequently expected to move and engage in physical tasks, including community outings
  • Occasionally required to lift or move items weighing more than 15 pounds
  • Must have the physical ability to stand for long periods of time
  • Must be able to intervene in the event of a physical conflict and provide assistance with personal care
