Arrowhead West is seeking a Case Manager who is passionate about empowering people with disabilities to live meaningful, productive, self-directed lives. In this role, you’ll support individuals in discovering strengths, setting goals, accessing services, and navigating the systems that help them live their best possible life
Salary & Benefits
• Starting at $20.50/hr.

What You’ll Get to Do
• Support individuals in making informed choices and achieving their goals.
• Coordinate services with programs, MCOs, and mental health providers, VR, medical providers, and community partners.
• Provide site visits, advocacy, and regular communication with individuals and families.
What You Need
• Bachelor’s degree and 6+ months experience with IDD/Intellectual and Developmental Disabilities OR equivalent experience in the field.
• Strong communication, planning, and documentation skills.
• Valid driver’s license and good driving record.
• Pass a background check and drug screening.
