Registered Nurse RN
Job Responsibilities: Promote and provide medical assistance in a professional manner, as needed, for healthcare, collaborating with physicians and multidisciplinary team members. Coordinate and supervise within assigned area(s) the medical and health care services provided to individuals served in accordance with federal, state, and local regulations and agency policies. The treatment of individuals served with dignity and respect is a prime responsibility.
Essential Functions
1. Participate in and/or provide information about the life planning process for individuals assigned to the caseload.
2. Maintain cooperative and professional relationships among teams and peers by communicating information, responding to requests, building rapport, and participating and contributing to team problem-solving methods.
3. Promote an individual’s independence by addressing medical treatments, assisting in understanding conditions, medications, and self-care skills, and answering questions.
4. Adhere to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations to protect individuals and employees.
5. Implement appropriate protective and preventative health measures for individuals served as per agency policy and other applicable regulations.
6. Oversee the safe and appropriate medication administration, storage, labeling, and record keeping.
7. Ensure that MARs match orders given by a medical provider.
8. Ensure that medications on site match orders and labeling.
9. Upload medical orders into Therap.
10. Provide instructional classes and curriculum for staff training, as designated.
11. Provide training to staff on topics related to specific medical needs. Maintain a nursing record that includes documentation of an individual’s care, treatments, and physician orders, and complies with DSNWK policies and federal and state regulations. This would include electronic updates to the person served information, medical supports, critical incidents, and progress notes.
12. Perform, document, and distribute medical screenings for persons served (weight, blood pressure, etc.). This may include Wellness Screenings for HCBS individuals.
13. As assigned, participate equally in the Weekend Nurse on-call responsibility with other agency nurses. Complete and submit Nursing On Call Documentation (ADM-047) daily or as needed for documentation of work after hours/weekends. Complete Nursing Weekend On Call Agreement CL-025h and submit to the Director for compensation for the weekend on-call stipend.
14. The nurse will arrange for coverage if the patient is unable to complete work or on-call duties during the weekday or assigned weekend on-call.
15. Maintain continuity of care among nursing teams by documenting and communicating actions, irregularities, and continuing needs.
16. Ensures the operation of medical equipment by completing preventive maintenance requirements, following manufacturer’s instructions, troubleshooting malfunctions, calling for repairs, maintaining equipment inventories, and evaluating new equipment and techniques, if applicable.
17. Be informed of the rights of individuals served and ensure these rights are maintained and protected.
18. Maintains a safe and clean working environment by complying with procedures, rules, and regulations; calling for assistance from health care support personnel.
19. Maintains nursing supplies inventory by checking stock to determine the level of inventory and needed supplies, placing orders, and verifying receipt of supplies as needed to accomplish job duties.
20. Provide oversight and consultation to LPNs as needed.
21. Delegate nursing activities to unlicensed assistive personnel.
22. Obtain and maintain Professional Liability Insurance and provide a copy of coverage to the Director.
23. Complete other duties as assigned by the Director of Programs and Services.
24. Maintain a current nursing license.
HIPAA Compliance
1. This position is responsible for maintaining confidentiality for all PHI and ePHI that is shared with this person. This position will have full access to PHI and ePHI of any individual receiving day and/or residential services from DSNWK.

1. Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.
2. Maintains confidence and protects operations by keeping information confidential.
3. Assures quality of care by making or recommending necessary adjustments to meet health goals, treatments, and standards of care set by the state board of nursing, the state nurse practice act, and other governing agency regulations.
