CDDO ASSESSMENT COORDINATOR: Serves as main point of contact for functional assessments for individuals/families receiving community-based services and supports for individuals with I/DD (Intellectual/Developmental Disabilities) within four county Community Developmental Disability Organization (CDDO) service area. Coordinates and facilitates annual assessments to capture functional abilities to maintain eligibility for HCBS I/DD waiver through review and interpretation of information from various resources and maintain accurate records in accordance with outlined timelines. Comprehensive training in I/DD assessment process will be provided. Must pass required training and completion certificate within 30 days of employment and annually thereafter. Minimum qualification include BA/BS degree in Human Services or related field of study and six months of experience with I/DD population or related field. May substitute six months of relevant experience for each semester of college with high school diploma or equivalent. Additional requirements include valid drivers’ license, three years driving experience and acceptable driving history. Must be able to meet all requirements as listed in the job description.
